Evans has all three (Moderna, Pfizer, and J&J) Covid-19 initial Vaccinations and boosters available. In the link below there is a form that will need to be printed and filled out BEFORE coming to Evans to receive your vaccine. If you are unable to print the intake form, Evans will provide one to you and it will need to be filled out before receiving your vaccine.
